Study Abroad Required Forms and Pre-Departure Handbook
UPEI highly recommends students stay enrolled in the UPEI health and dental plan while studying abroad
If you are covered under the UPEI Student Union plan, please contact the Students’ Association or the PBAS Group to obtain a 365-day Medical Assistance Travel Card.

If you opt out of the UPEI Student Union plan, you must show the Office of Study Abroad and International Partnerships your proof of coverage from your private insurance provider that states the exact dates you are covered for emergency medical abroad (this must be for the entire duration of your stay).
The following forms are required for those participating in UPEI study abroad programs unless otherwise noted.
UPEI Education Abroad Forms
The UPEI Education Abroad Forms need to be reviewed, filled out, signed, and then returned to the Office of Study Abroad and International Partnerships. Completed forms should be emailed to studyabroad@upei.ca.
The forms contained in this .pdf file include:
- Release of Liability, Waiver of Claims, Assumption of Risk and Indemnity Agreement
- Supplemental Travel Waiver
- Risk Management Form
- Emergency Contact Form
- Financial Statement (optional, but recommended)
UPEI Emergency Contact Card
Please print or save the UPEI Emergency Contact Card with your documents in case of emergency.
UPEI Letter of Permission (LOP) Form
Note: this form is NOT required for those taking a UPEI faculty-led course.
The UPEI Letter of Permission (LOP) Form must be completed with courses you plan to take while abroad. The completed LOP form ensures you get the courses added to your UPEI transcript once you are done with your studies abroad.
Enter the courses you plan to take abroad and the equivalent UPEI courses (sometimes these will need to be listed as UPEI elective courses - check with the department chairs/deans if you need any assistance). The LOP form gets signed off/approved by the relevant UPEI department chairs/deans for each course. Once you have department chair/dean approval, the form is sent to the UPEI Registrar’s Office for final approval. Upon completion of your study abroad experience, provide the UPEI Registrar's Office with the transcript from your partner school upon completion of your studies abroad.
Yes, you can adjust your LOP form if you have a course you want to add/drop once abroad! Just fill out a new form for the new course, get it signed and submitted to the Registrar’s Office. Make sure to let the Registrar’s Office know if you’ve dropped a course!
